Mitchell51 states that patch testing with mixes can provide requisite information. Thus, in a patient with clinical findings strongly suggestive of rubber glove dermatitis, a positive reaction to thiuram mix leads to advice on avoidance of rubber, and further testing may be unnecessary. The multitude of allergenic rubber chemicals leads us to test them in groups because it would require too much effort to test each substance individually.
Eosinophils may be found in both the dermis and the epidermis and, if present, suggest ACD rather than another form of dermatitis. Starting at about 24 hours, and perhaps even earlier, basophils begin to appear. They may comprise up to 15% of the infiltrate by day 3, at which time tissue mast cells begin to increase in number. Whereas basophils enter the site from the blood, mast cells are formed by local replication, accounting for their delayed appearance. Other changes within the 24- to 48-hour time frame may include increased vascular permeability resulting in rare erythrocyte extravasation and dermal papillary edema, as well as fibrin deposition in the reticular dermis.
Many studies were based on this series, and it is included here for historical purposes. ’’ The series may be general, that is, a ‘‘screening’’ group, or specific, such as a ‘‘therapeutic,’’ ‘‘metal,’’ or ‘‘rubber’’ series (Figure 2-8). Such groupings often overlap because the screening series may contain metals, rubber chemicals, and topical therapeutic agents that are common sensitizers. E. test system. As noted earlier in this chapter in the section ‘‘Interpreting Patch Test Reactions,’’ larger screening series capture more relevant information.
